Complete the 4506-T EZ
Both forms must be signed and dated along with a daytime phone number. (Complete lines 1-4 and note in line 9 (2) recent tax years.)
Tips on Completing the 4506-T:
- Complete lines 1-4 of the document
- Leave lines 5,6,7,8 blank
- On line 9 enter the years of the (2) two most recent tax returns (for example “2010″ and “2011″)
- In the signature section, be sure to place your “telephone number of taxpayer” in the appropriate space provided
- Be sure it’s signed and dated.
